Please provide some more info:
Age and size of tank?

My tangs did this. I bought a cleaner shrimp and the problem went away. It could be a parasite or something irritating him.

"filtration: just a protein skimmer"
Did you know that's only ~10% of the total filtration?

"alk/cal 450"
Assuming that is 450 ppm Ca, what is the Alkalinity?

"the fish has being in the tank for about a month"
Does the Cleaner Shrimp do any "cleaning"?

well for the filtration i also have the live rock and live sand
im not sure about the alkalinity i got it tested like 5 days ago but i forgot what the number was
the cleaner shrimp does clean him once in a while but the tang doesnt really like it i did notice the tang swiming by the shrimp but i guess it was after hours because the shrimp didnt clean him, anyway the tang is looking better now and its eating alot he takes algae and pieces of shrimp of my hand

That's good.
I cannot stress enough how important it is to know the Alk and Ca levels. Smile

I know it's kind of a tease, but when I ask about filtration it gives me an idea of that hobbyists true understanding of what their tank is all about. Ideally what I hope to see is the type & depth of LS, amount of LR and growth of any algae(including Coralline) in the tank or Refugium. This is information that paints a good picture when a real pic is not available.

Most of the time, when an animal is not eating what it is supposed to, like a Shrimp that isn't cleaning/eating parasites off a fish, it's because it is being overfed on other foods.
